Windshield Repair Authorization & Disclosure

1. Authorization of Work — I hereby authorize Rapid City Windshield Repair to perform professional windshield repair services on my vehicle. I understand this process involves cleaning the damaged area and injecting a specialized resin to restore structural integrity.

2. Disclosure of Risk ("The Spreading Clause") — Windshield repair involves applying pressure to a damaged and weakened area of glass. While our technicians use professional tools and techniques, the act of repairing a chip or crack can occasionally cause the damage to expand or "run."

• Liability Release: I acknowledge that Rapid City Windshield Repair is NOT responsible for the cost of a windshield replacement if the repair fails, if the damage spreads during the repair process, or if the damage spreads at any point after the repair is completed.

3. Visual Expectations — The primary goal of a repair is to restore structural strength and prevent the damage from spreading. While the visibility of the chip or crack is usually reduced by 60-90%, a repair is not a total disappearance of the damage. A small scar or mark may remain where the resin was injected.

4. Limited Warranty & Refund Policy — If the repaired portion cracks further through the specific area repaired by Rapid City Windshield Repair, we will refund the original cost of the repair or credit it toward a future service. This warranty does not cover glass replacement or recalibration.

5. Consent — By checking below, I confirm I am the owner or authorized agent of this vehicle and have read, understood, and agree to the terms above.
